The West Moreton Older Persons Care Collaborative

The West Moreton region is located approximately 50km west of Brisbane in Queensland. The region has a total population of over 290,000 people and covers an area of 9,521 square kilometers from Esk in the north, Gatton in the west, Ipswich in the east and Boonah in the south. A significant challenge for West Moreton is the expected 43% increase in the population from 2016 to 2026 and the 113% increase to 593,000 by 2036. Furthermore, the population of older people in the West Moreton region is anticipated to grow by 83% in the next 10 year.

The West Moreton Older Persons Care Collaborative (OPCC) was established in July 2021 with the aim of developing partnerships across government and non-government services in order to optimise systems, processes and outcomes for older people in the West Moreton community. The collaborative was established through a partnership between Darling Downs and West Moreton PHN and West Moreton Health. The OPCC is supported by University of Southern Queensland (USQ) in partnership with the Darling Downs and West Moreton PHN.

Action Plan

Older Persons Care Collaborative

The action plan, finalised in April 2022, sets the direction and work plan of the OPCC for the 2022-2023 period. The networks within the collaborative are achieving the goals by identifying gaps in care systems and needs of older people within their communities of practice, and developing strategies (actions) to address those gaps. The networks are supported by the overarching governance committee.

At the time of development, the OPCC has two networks, each with their own set of goals, actions and strategies:

  • Community and Rural Network
  • Dementia Network

The goals of the governance committee are:

  • To achieve equitable access to support and quality care for all older people in the West Moreton community, regardless of setting, diagnosis, location, disability status, socio-economic status or social circumstances.
  • To embed a culture of evidence-based actions, and evaluation in to the core functions and actions of the OPCC
  • To optimise the sustainability of the OPCC
  • To raise community awareness of supports and services within the West Moreton region for older people to live as well and healthy as possible
  • To support the workforce supporting older people in the region with education
